A pre-demolition asbestos survey was completed on each parcel by Industrial Insulation Specialists and their survey results are included as Attachments A-C of this RFP.

Contractor Requirements:

Contractor must be licensed and certified by the State of Ohio to conduct asbestos abatement projects. All persons working on any asbestos abatement project on Contractor's behalf must also be licensed and certified by the State of Ohio.

 

Scope of Services:

Contractors interested in obtaining a contract with RCLRC for providing asbestos abatement shall prepare a written proposal to include, but not limited to, the following:

  • A quote on the removal of the asbestos identified in the asbestos survey repmis (Attachments A-C).
  • Removal and disposition of asbestos shall be in accordance with industrial standards as well as all Federal, State, and local regulations.
  • All materials identified as asbestos containing must be removed and disposed of by properly trained personnel.

 

Insurance:

Contractor and all tiers of Subcontractors shall at all times during the period in which the Contract is in force and effect, provide and maintain the following insurance at their own expense. All insurance shall be purchased from insurance carriers authorized to do business in the State of Ohio. All insurance required herein shall be written on an "occurrence" basis and not a "claims-made" basis. In no event shall work be performed until the required evidence of insurance has been furnished. If Contractor fails to obtain or maintain the required insurance, the RCLRC shall have the right to treat such failure as a material breach of contract and to exercise all appropriate rights and remedies. The insurance shall provide for at least thirty (30) days prior written notice to be given to the RCLRC in the event coverage is materially changed, canceled or non-renewed.

The Contractor shall not commence work under this contract until he has obtained all insurance required herein and such insurance has been approved by the Owner, nor shall the Contractor allow any subcontractor to commence work on his subcontract until all similar insurance required coverage of the subcontractor has been so obtained and approved.

Compensation Insurance. The Contractor shall take out and maintain during the life of this contract, Workmen's Compensation Insurance for all of his employees employed at the site of the project and, in case any work is sublet, the Contractor shall require the subcontractor similarly to provide Workmen's Compensation Insurance for all the latter's employees unless such employees are covered by the protection afforded by the Contractor.

 

Comprehensive Liability Insurance. Indemnification: The contractor shall fully indemnify and save and hold the RCLRC free and harmless from and against any and all claims, damages, losses, liability, and judgments for personal injuries (including death) and prope1iy damage, whether occurring to the persons or properties of the Contractor, his agents or employees, or of subcontractors, if any, or their agents or employees, or any other persons, including the city, arising out of or in connection with the work called for by the contract, or occurring by reason of, or resulting from or occasioned or caused in whole or in part by, any act or omission of the Contractor, any subcontractor, anyone directly or indirectly employed by any of them or anyone for whose acts any of them may be liable, or (as to personal injuries, including death, to the Contractor or his employees, or subcontractors or their employees) of the RCLRC or its agents or employees or of any other person (s), whether in the execution or in the guarding of the work or otherwise, and free and harmless from and against all claims for payments of Workmen's Compensation premiums (other than the RCLRC's) including the defending of any and all litigation with regard to any of the claims referred to anywhere above in this paragraph. In any and all claims against the RCLRC or any of its agents or employees by any employee of the Contractor, any subcontractor, anyone directly or indirectly employed by any of them, or anyone for whose acts any of them may be liable, the indemnification obligation under the


next preceding paragraphs shall not be limited in any way by any limitation on the amount or type of damages, compensation, or benefits payable by or for the Contractor or any subcontractor under Workmen's Compensation acts, disability benefit acts, or other employee benefit acts.

 

Liability Insurance. The Contractor shall maintain such insurance as will protect him from claims under Workmen's Compensation acts and from any claims for damages for personal injury, including death, which may arise from operations under this contract, whether such operations be by himself or by any subcontractor or anyone directly or indirectly employed by either of them. The Contractor shall provide original ce1iificate of insurance naming Ross County Land Reutilization Corporation as additional insured.

 

The Contractor shall secure and maintain insurance of and in the following types and amounts.

 

(i)                   Comprehensive Liability. (to also include contractual protection covering the liability assumed by the contractor under the terms of this article.)

Content and Format of Proposals

The proposers interested in this project shall submit one (1) printed copy of the proposal package to meet the requirements below. The proposal package shall include, but not be limited to, the following:

  1. Provide an itemized fee schedule for the requested services.
  2. Provide a written statement outlining the timeline as well as an estimated completion date.
    1. Provide license numbers or copies of registration ce1iificates, as appropriate, for asbestos abatement supervisors and workers.

-Asbestos Abatement" Each Proposal must contain the full name of the party or parties submitting the Proposal and all persons interested therein. No Proposer shall withdraw his Proposal after the actual opening thereof. Ross County Land Reutilization reserves the right to reject any or all bids, waive inegularities in any Proposal, and to accept any Proposal that is deemed by the Corporation to be in the best interests of the Corporation.

SCOPE OF WORK

Contractor shall establish a contained work site as required by 29 C.F.R., Part 1926.1101 at the Site. Contractor shall be responsible for proper abatement and disposal of any asbestos containing material as identified by the attached asbestos report.

CONTRACTOR MUST PROVIDE OWNER WITH LANDFILL RECEIPTS FROM ABATEMENT.

Contractor will provide, if required, a negative pressure air filtration system to be operating during the hazardous materials activities. Provide final clearance air sampling upon completion of hazardous materials abatement. Contractor will provide Demolition Company and/or RCLRC with a ce1iificate of clearance as prescribed by law stating that the Building if free of all hazardous materials and legally suitable for demolition. Contractor will accomplish the disposal of all hazardous materials-accompanying debris to an EPA-approved landfill with copies of all manifests, transit and trip receipts confirming chain of custody of all hazardous materials.

Contractor proposes and agrees to furnish all labor, supervision, expe1iise, material, equipment, transpmiation and disposal necessary to complete all asbestos remediation work for RCLRC as described in the pertinent Asbestos Survey/Inspection Report (Attachments A-C) for the fixed price stated in this bid proposal.

Contractor will complete Notification of Demolition and Renovation and submit to the Ohio Environmental Protection Agency. Cost must be included within contractor's proposed bid.

Contractor will communicate with Demolition Contractor any and all pe1iinent information regarding the asbestos removal and notification to the Ohio Environmental Protection Agency.

PROJECT DATE(S): Commencement and completion of work shall be completed on all properties listed herein no later than March 25, 2019.

DRUG FREE WORKPLACE COMPLIANCE

Ross County Land Reutilization Corporation is a recipient of federal funds and is required to comply with the Drug Free Workplace Act of 1988. The RCLRC is concerned with the effects drug or alcohol abuse can have on employees, their families, and the employee's ability to perform their work safely and efficiently.

Before RCLRC enters into a contract for the purchase of materials, equipment, supplies, contracts, or insurance, or other services, the contractor must agree to the following:

 The Ohio EPA requires 10 working day prior notifications for any structures that are scheduled to be demolished or burned.

 NESHAP category I & II ACMs must be removed from the designated building prior to demolition. The removal of any ACMs must be performed by a licensed Asbestos (Ohio EPA) contractor.

During Demolition.

 When Demolition by toppling occurs, adequate wetting must be employed to suppress the dust and reasonable enclosures for dust emission control may be employed if compatible.

(5) Survey Methods

The sampling protocol used in this asbestos survey is listed in the Tile 40 Code of Federal Requlations(CFR), Part 763 for suspect ACMs. The EPA requlations require that sample locations be randomly selected. Suspect asbestos-containing building materials were identified and sample of each different type material were obtained.

 The bulk sampling procedure utilized for collection of samples suspected of being ACMs required the establishment of a homogeneous sampling areas.

A homogeneous sampling area is defined as an of friable or non-friable materials of similar type that appeared to be applied or constructed during the same general period of time. This the most acceptable and cost effective method for sampling of suspect ACMs building materials.

Materials visibly and completely identifable as non­ asbestos,(wood,rubber,glass,metal,foams,fiberglass and etc) were not sampled.

Laboratory Analysis

Samples are collected and sent to a 3rd party Lab. Bulk sample were analyzed by polarized light microscopy (PLM) using EPA method 600-R93-116 incorporating visual estimate of identifiable material percentage. Any material that contains less than one percent(<1%) may be left in place during Demo.

 It should be noted that 1.1.S does not core sample roofings products to aviod creating roof leaks. Roofing materrials and others suspect ACMs found behind walls or hidden cavites should be assumed or not to contain ACM and handled and disposed accordingly.

 The results and finding and conclusions expressed in this report are based  only on condition observed during our survey of the accessible areas of  this building(s). 1.1.S in this report makes no assumption as to the past conditions or future occurrence. The finding of this report represents our professional judgement, no orther warranty is expressed or implied.

 This report is prepared to assist the building owner, architect, construction manger, general contractors and potential asbestos contractors in locating ACM. This report is not intended as a bidding document or as a project specification.

(7) Landfill and Waste

 Non-friable ACM debris dislodged during demolition can be disposed off in a sanitary landfill that accepts ACM demolition and constrution debris waste within the framework of local/state regulations. Please check with your landfill regarding their policy on acceptance of such waste,prior to demolition project.

 Non-friable ACM debris mixed with demolition debris should not be used as fill material on-site nor should it be sold or given away.

 If the demolition contractor changes the means and methods of the demolition and the environment consultant it the opinon that Category I non-friable materials are being made friable, or visible dust emissions are being generated, the project may be stopped.

 In these situation a revised notification for removal/clean up of non-friable ACM may be necessary and the removal work will then be needed to completed by a Ohio EPA licensed abatement contractor.

Asbestos in basement ductwork (100' less that 'If) must be removal be for Demolition.

Prior to Demolition.

The Ohio EPA requires 10 working day prior notifications for any structures that are scheduled to be demolished or burned.

NESHAP category I & II ACMs must be removed from the designated building prior to demolition. The removal of any ACMs must be performed by a licensed Asbestos (Ohio EPA) contractor.

During Demolition.

When Demolition by toppling occurs, adequate wetting must be employed to suppress the dust and reasonable enclosures for dust emission control may be employed if compatible.

(5) Survey Methods

The sampling protocol used in this asbestos survey is listed in the Tile 40 Code of Federal Requlations(CFR), Part 763 for suspect ACMs. The EPA requlations require that sample locations be randomly selected. Suspect asbestos-containing building materials were identified and sample of each different type material were obtained.

The bulk sampling procedure utilized for collection of samples suspected of being ACMs required the establishment of a homogeneous sampling areas.

A homogeneous sampling area is defined as an of friable or non-friable materials of similar type that appeared to be applied or constructed during the same general period of time. This the most acceptable and cost effective method for sampling of suspect ACMs building materials.

Materials visibly and completely identifable as non­ asbestos,(wood,rubber,glass,metal,foams,fiberglass and etc) were not sampled.

Laboratory Analysis

Samples are collected and sent to a 3rd party Lab. Bulk sample were analyzed by polarized light microscopy (PLM) using EPA method 600-R93-116 incorporating visual estimate of identifiable material percentage. Any material that contains less than one percent(<1%) may be left in place during Demo.

(6) Assumption and Limitations

It should be noted that 1.1.S does not core sample roofings products to aviod creating roof leaks. Roofing materrials and others suspect ACMs found behind walls or hidden cavites should be assumed or not to contain ACM and handled and disposed accordingly.

The results and finding and conclusions expressed in this report  are based only on condition observed during our survey of the accessible areas of  this building(s). 1.1.S in this report makes no assumption as to the past conditions or future occurrence. The finding of this report represents our professional judgement, no orther warranty is expressed or implied.

This report is prepared to assist the building owner, architect, construction manger, general contractors and potential asbestos contractors in locating ACM. This report is not intended as a bidding document or as a project specification.

(7) Landfill and Waste

Non-friable ACM debris dislodged during demolition can be disposed off in a sanitary landfill that accepts ACM demolition and constrution debris waste within the framework of local/state regulations. Please check with your landfill regarding their policy on acceptance of such waste,prior to demolition project.

Non-friable ACM debris mixed with demolition debris should not be used as fill material on-site nor should it be sold or given away.

If the demolition contractor changes the means and methods of the demolition and the environment consultant it the opinon that Category I non-friable materials are being made friable, or visible dust emissions are being generated, the project may be stopped.

In these situation a revised notification for removal/clean up of non-friable ACM may be necessary and the removal work will then be needed to completed by a Ohio EPA licensed abatement contractor.

1.1.S has completed the pre-demolition survey for asbestos-containing materials (ACM) at the above- referenced location. This report/survey includes the procedure and methodologies followed, analytical laboratory and applicable conclusions and recommendations.

69 1/2 Hickory( Unit 2) Street Chillicothe, Ohio has abestos tape on the duct work and it must be removed be for Demolition. A notification of Demolition and Renovation/Abatement form must be sent to the Ohio EPA, at ( Ohio EPA P.O. 1049 Columbus, Ohio 43216).

Roof, flooring and other non-friable materials are considered EPA NESHAP Category I non-friable ACMs and according to the EPA, may be left in place during the scheduled demolition of this particular structure.
